If you have been living with a denture for some time you know what it is like; there is no more toothache and cleaning but there are other and sometimes more serious problems:

  • You have a plastic thing in your mouth, which is very difficult to get used to. Soft tissues in the mouth get harder and sore spots develop. With an upper denture the roof of your mouth and the taste buds there are covered, you cannot feel and enjoy food as before.
  • Chewing force of a denture is only a fraction of that of fixed teeth and if you cannot chew food well your nutrition and digestion is weaker.
  • When you wear dentures for a long time the bone gradually recedes because it is not supported by the teeth therefore it is not used. The denture becomes loose and floppy as you have less and less bone to support it.
  • As the bone recedes your face changes: the distance between your nose and your chin gets smaller, your lips collapse making wrinkles on the two sides of your mouth.

Comfort Control Syringe

comfort_control_syringeIn the dental surgery most people are worried about the needle and the injection more than anything else and this is absolutely reasonable: the injection is the only thing you feel; after that your mouth gets absolutely numb. There are certain products that make injections less unpleasant, like anaesthetic gels, which the dentist applies on your gum to freeze it, but still you feel the needle and the pressing flow of anaesthetic material into the tissue… but this may also be the past.

The latest comfort control electronic syringe has a very tiny needle, much smaller than normal syringes and provides a flow of anaesthetic ahead of the needle – so that you can hardly feel it at all. When the needle is in it provides a slow and steady flow of anaesthesia into the tissue, which is absolutely pain-free, allows the dentist to set a constant level of anaesthesia and keep a continuous control over it.

So instead of having a normal needle and a large amount of anaesthetic material in one go, you will get the injection with a much smaller needle and over a long period of time. Once you have experienced this new type of injection you will be even more relaxed at the dental visits.

I’ve just got back from four days in Budapest…with a new set of teeth – gleaming quietly as I write – and a rather fatter bank balance than I feared I would have a couple of months ago, when my growing need for quite drastic dental treatment became only too obvious.

English price, for implants, crowns and a bottom bridge Circa £6500@:Hungarian price Circa £2000…plus, courtesy of Easyjet, a package of £250 for flights and hotel, plus quite lavish living for four days in Budapest, at £200. Well under half price.

And, very much to my nervous surprise, the whole process was virtually painless. Furthermore, I can honestly say that results are already making me feel a lot better about myself a: no spitting, no tendency to dribble, no hasty swallowing of half chewed mouthfulls, and noticably better digestion… much reduced rumbling, farting and belching !

Hungary is quite famous for dentistry. The current Bradt Guide sums up the position quite clearly “If you require complicated dental work, that goes beyond the standard clean-up or filling, the economic argument is overwhelming, with procedures costing at least half – and sometimes as little as a quarter – of the price paid in the UK. ” The guide lists a tariff.

Its early days to be 100% secure about the enduring quality of the work I’ve had done, but my dentist has full insurance with the UK Medical Protection Society, and my Titan Implants are guaranteed. So it really is hard to see the arguments against this simple to achieve, and very cost effective ‘arbitrage’. Half the price and just as good.

I had four treatment sessions, which followed a plan which was developed following a consultation and X-ray session in the Hungarian practice’s London centre, which – cleverly – operates in a large West London Dental surgery, which is theirs on Saturday’s and Sundays, while the British professionals are enjoying their week-ends at leisure. Obviously the process of extraction, root surgery and the preparation and impanting of the hardware, isn’t a bundle of fun, but there was no pain, only a mild discomfort as the numbing effect of the anaesthetics wore off. Ice packs, painkillers and prophylactic antibiotics are part of the package. After my evening sessions I slept well and woke pain free, and after morning sessions I visited the Museums and Art Galleries….a tribute to the gentle and expert ministrations of the dental surgeons.

And there are cosmeticians and technologists to advise on the appearence of the work. No flashing pearly white flawlessly even perfection for this 70 year old ! Mildy baroque and creamy….good for eating and smiling, as the white coated mature boss lady of the specialist technician team put it, after colour matching the new to the old.

On top of all this, the Hungarian cultural experience in Budapest is more than satisfactory. The excitement of unexplored treasure houses, in a World Heritage setting, five minutes away from one’s hotel door, certainly tends to take one’s mind off the invasive nature of the dental treatment !

I opted for January, the lowest of the low season, but therefore had quiet and almost exclusive enjoyment of the wonderful National Gallery collection. And my flight/hotel package was cheap. If I’d opted for April, the Opera and Concert season would have been enticing, and the prices higher – dentistry apart, of course. My four star hotel was literally a stone’s throw from the the mighty Danube and the Chain Bridge, and equi- distant from the furnicular to Castle Hill….so one was an easy stroll from the best bits of both Buda and Pest.

You need to know the most important detail in detail let us emphasize the most important point: as soon as you become aware that the root of any of your teeth is broken or fractured, please visit the dentist at your earliest convenience, even if the tooth does not cause any serious pain.

And now, here are the explanations and reasons: unfortunately it sometimes happens that a root of a tooth gets broken or fractured and in this case there is no way to save or repair it; extraction is the only possible solution. The fracture creates a channel for bacteria from the mouth to the root and the jawbone, causing continuous and more and more serious infection in the bone. Antibiotics can give you temporary relief from the symptoms but if you feel you gain some time with the help of medications you should know that you lose more than you gain…

The jawbone is not innervated with sensitive nerves therefore you do not necessarily feel serious pain, still you will surely feel that „something is not right” with that tooth. The infection constantly suppresses the immune system, and as a consequence of that you may have some other symptoms that you would never guess to be related to your teeth: loss if hair, pain in the joints or frequent smaller diseases.
While the continuous inflammation puts the body under constant pressure, the bacteria in the bone cause further serious damage: they destroy the jawbone to reduce the chances that the tooth will be able to be replaced with a dental implant. There is a minimum necessary width, height and density requirement for the jawbone, which the dentist has to measure and examine before dental implants can be placed – therefore it is absolutely necessary to keep the jawbone as healthy and strong as possible.

And now for you to see what the novelty is in the traditional implant procedure the dentist first extracted the jawbone and recommended 3-6 months healing time before the dental implant is fitted. During the healing time the bacteria cleared from the jawbone and the bone-building tissues restored the jawbone at the place of the extraction. After the recovery period in most cases the bone recovered enough to be able to support the dental implant but there was one disadvantage as well: the gum lost its original shape and became „flat” following the line of the gum. When the crown was prepared at the final stage there was no way to restore the original and natural oval-shape gumline at the top of the crown.

With the latest implantation method the implant is fitted immediately after the extraction; when necessary the dentist adds artificial bone for extra support. With this technique the bone will not collapse as it did before as the implant and the artificial bone together keep the original bone level. Whenever possible, a plastic temporary crown is fitted on the implant at the same time, which is formed very thoroughly to make sure that the temporary crown follows the natural gumline.

One great advantage of the new technique is that the patient does not have to struggle with the temporary denture for months; in most cases some fixed solution can be provided immediately after the extraction. If not, the healing period is reduced so you have to put up with the temporary solution for a shorter time.
The other advantage is also obvious: the plastic temporary crown will preserve the oval-shape gumline and when the permanent crown is prepared, the new tooth will look absolutely natural.

The condition of success is that there should not be any infection at the root of the broken tooth; bacteria should have no time to attack the jawbone and destroy it. Even after a few months serious infection can occur and hinder reaching the perfect result… although this is what we always aim for!

Fixed crowns and bridges

What you should know before your treatment starts

When preparing crowns and bridges the dentist shaves off the surface of the teeth and takes impressions for the dental technicians, who will prepare the new teeth.

Crowns and bridges are prepared in a dental laboratory in 3-5 working days. For the period temporary plastic crowns /bridges are prepared for you. Please remember that these temporary solutions are not similar to or equivalent with the final result either from aesthetical or functional aspect. As it is prepared from liquid plastic the dentist can reconstruct the shape of the previous teeth and we can provide the temporary teeth only in the shades that are available; there is no such a wide range of shades and transparencies as in case of porcelain. Under the temporary crowns and bridges the teeth may be sensitive; please bite and chew with them very carefully because they may come off or chip off. When the permanent crowns and bridges are fitted the dentist has to be able to remove the temporaries easily, without damaging the tooth underneath; this is why the temporaries cannot be fixed with some strong type of cement.

In case the temporary crown or bridge comes off.. Please contact your agent immediately so that arrangements can be made to refit or replace it. If you find it easier to replace it yourself, please never use any superglue as they contain dangerous acids. What you can safely use to refit the temporary crowns or bridges until you can see the dentist is any type of strong denture glue. Please also note that the temporary crowns do not cover and seal the tooth surface perfectly so they can be used only for a few weeks; after that the tooth may decay under them.

In some cases you will be asked to see technicians in order to select the best matching shade for your new crowns and bridges. The agent will drive you to the laboratory and assist you in the process. The technicians will be happy to meet you there and choose the ideal shade with the shade guide they use. In case you have any special requirements, ideas or requests with regards to the shade or the shape of your new teeth, please feel free to discuss it with them and also with the dentist. In case of full front reconstruction you may prepare with some photos or pictures to show us what Perfect Smile means for you. The shape and the form of your new teeth is mostly determined by your bite, the size and structure of your jawbone and many other factors but we will do our best to take your expectations into consideration and meet your requirements.

The new crowns and bridges can be fitted with temporary or permanent cement. In case you had extraction before the bridge was prepared, the new bridge will be fitted with temporary cement in any case as the bone level will change at the place of the extraction and settles down only after 3-4 months. You may feel some gap between the bridge and your gum during the healing time; In this case, patience is needed because the bone level constantly changes and the dentist can make the final correction only when the healing is over. We expect to see you in the surgery at the time agreed, the dentist removes the temporarily fitted bridge and if necessary, takes it back to the dental laboratory for the corrections; the technicians will adjust the bridge to the actual gumline.

The other advantage of temporary fitting is that any aesthetical or functional problems can be corrected at a later date, when the temporary cement has dissolved. Temporary fitting lasts for about 3 months in case of bridges, and 1-1.5 months in case of crowns. During this time you have the opportunity to decide if the new teeth perfectly meet your expectations and if you require any corrections (shade, form, etc.) they can be done in the dental lab. Temporary fitting is suitable only for about 6 months; after that time please contact us for an appointment when permanent fitting and possible corrections will take place.

In case the crowns and bridges are fitted with permanent cement, they can be removed only with dental instruments by cutting them off. In most cases it is necessary only after 10-20 years when they are due to be replaced.

Information after the treatment

After fitting the new crowns and bridges the dentist adjusts the bite so that biting and chewing should be absolutely comfortable. At the same time only you can exactly feel if the new tooth perfectly fits, if you feel anything unpleasant. If you had your teeth missing for a longer period, probably it will take some time until you get used to the new tooth; in the first few days it will surely feel strange even for your tongue. Some sensitivity under the new crowns or bridges is also normal; it will completely disappear in 1-2 weeks’ time.

Please let your agent know and arrangements will be made for you to return to the surgery for further adjustments and corrections.

Occasionally it happens that after the preparations the tooth’s nerves may die; in this case root canal filling becomes necessary to prevent any further infection.

Temporary fitting is suitable only for about 6 months, after that time please contact the surgery for an appointment when permanent fitting and possible corrections will take place.

In case you feel that your teeth get sensitive under the new crown/bridge or you feel any slight movement it shows that temporary cement has dissolved and the crown or bridge can be fitted permanently. Before the permanent fitting please let us know of you wish to get anything adjusted or corrected (shade, colour, etc.).

In case the crowns and bridges are fitted with permanent cement, they can be removed only with dental instruments by cutting them off but smaller adjustments and corrections can be done in this case as well.

Crowns should be cleaned just as normal teeth but bridges require some special way of cleaning. Special flosses and toothbrushes are available especially for bridges and using antibacterial mouthwash is also advised. Still the ideal way of cleaning natural teeth, crowns and bridges is using a water flosser or oral shower as it sprays water on your teeth with high pressure and removes any remaining food from between your teeth and under the bridge.

All crowns and bridges should be checked by your dentist at least once a year.
